Please review the patient permissions, financial, and HIPAA file which is accessible here. When you come into our office, you will sign an acknowledgement that you have been offered a copy of these permissions. Please also make sure that we have all pertinent medical records on file before coming in for your appointment. Call your referring physician to assure we have your referral sent to our office fax at 1-855-383-3233. Also, make sure to call us to update your insurance ahead of time if your insurance has changed. Lastly, bring all updates to your medical history and a current list of medicines that you take.

Below is a check list for you to remember to bring with you.

-Check that we have your current insurance

-Call us to update your insurance ahead of time

-Bring your insurance card and photo ID with you

-Review patient permissions file

-Send medical records to our office

-Bring list of current medicines, surgical history, family medical history

-If cancelling or rescheduling, make sure to cancel your appointment at least two business days prior to avoid our $50 cancellation fee
